 Please could you say how many times the trust has released a patient's details to any external public body (eg. councils, police forces, etc) without receiving the relevant patient's consent, in each of the last three years. In each case, please state:- which public body received the information- what exactly was released (eg. names, addresses, DOBs, case histories, letters, results, nursing records, X-rays, physiotherapy records, A&E records, etc)- what exactly was the reason stated by the external public body (eg. court order relating to child protection case, fostering cases, council investigation into care homes, etc). Please give as much detail as possible to avoid me pestering you again in a month's time!- please say whether the transfer of data had to be approved by the trust's Caldicott Guardian

The number that provide out of hours appointments/walk ins – please provide details of surgery addresses The following questions relate only to Out of Hours surgeries and prescriptions written with medication provided during this visit covering April 07 – April 08: 3. The number of patients prescribed and provided medication during the visit over a period of 1 year (April 07 – April 08) 4. The number of patients who were eligible for free prescriptions 5. The amount (in number and value) of invoices raised to collect these funds 6. How many surgeries take payment on site – if so in what form of payment – cash/cheque/card 7. The amount of invoices paid in 30 days, 60 days – later than 60 days 8. The number and amount (in value) of invoices that remain unpaid over the year 9. The number of staff processing invoices and chasing payment  10. The policy for chasing unpaid invoices

 Does the PCT have a specific policy with regard to prescribing contact lens solutions for Keratoconus? Please let me have a copy of the policy, details of the ophthalmic qualifications of the people who framed the policy.2) If the PCT does not have a specific policy, but uses some wider guideline, please let me have a copy of that guideline. 3) Please can you send me a list of NHS establishments in your PCT area that include corneal care among the services that they provide.

 Could you please provide me with a list of treatments your trust has been unable to fund, ceased to fund or restricted for financial reasons in the last year from today's date? Could you please provide me with a list of treatments your trust is currently proposing not funding, ceasing to fund or restricting for financial reasons? Could you please provide me with a list of treatments your trust anticipates needing to restrict or ceasing  to provide in order to fund extra end of life cancer drugs costing above  the £30,000 QALY  threshold as  stated in the new NICE guidance to its appraisal committee on appraising treatments which may extend life at the end of life?  Could you please provide me with any documents, published in the last year from this date, in which your trust prioritises offering treatment to patients with particular lifestyles such as smokers or non-smokers, the overweight or those of a normal weight, those whose alcohol consumption is within or above recommended levels? Could you please provide me with any documents, published in the last year from this date, in which your trust considers giving priority for treatments to patients of a particular age?

 I was hoping I could find out if any of the agency positions mentioned were linked to the plans to move consultant-led maternity services from Eastbourne to Hastings and if I could get a brief description of the tasks/projects each member of staff completed. Could I find out if any were covering for a vacant permanent staff post and if so, how much an employee of the trust in that post would earn as a salary. Also, how much were each of the three people above paid in total, how much of their wage was taken by the agency itself and how much was spent on agency staff in the first two quarters of 2008/09 in total. As background, could you tell me Mike Wood's salary band, Ken Ellis's, and that of a consultant, GP and junior doctor working for the trust. I assume these are made public, as ESHT's are.

 We are compiling details of how treatment that trans people receive varies across Primary Care Trusts.  I should be grateful of you would please comment generally on how closely your Trust follows the World Professional Association for Transgender Health's (WPATH) Standards of Care for Gender Identity Disorders.  The specific areas that I am looking to compare across the PCTs are the availability of the following services, based on clinical or psychological need – clearly, not every patient will require every treatment.  Counselling, Hormone prescriptions, Referral to Gender Identity Clinic, Patient choice of Gender Identity Clinic, Voice therapy, Voice surgery, Mastectomy, Hysterectom,y Phalloplasty, Penectomy, Vaginoplasty, Breast augmentation, Hair removal (laser or electrolysis), Orchidectomy, Feminising facial surgery.  I should also be pleased to know the Standards of Care applied in respect of children and adolescents with Gender Identity issues, including social, psychological and physical interventions.

 What is the population size served by your PCT? 2) Please give details of the provider(s) of your medical out-of-hours service(s) (If you have separate contracts for call centre services and provision of medical care, please list both) 3) If you commission jointly with other PCTs please list the  associate commissioners and the co-ordinating (lead) commissioner 4) What is the value of your out-of-hours contract(s) for your PCT population in 2008/09? 5) What is the duration and end date of your out-of-hours contracts? 6) Do you intend to tender for the contract(s) when it/they end? 7) Do you intend to tender for any other primary care/urgent care services requiring call centre services as part of the specification within the next 12 months? If so please give details (examples might include GP led extended hours practices, Walk-in centres, A&E front end services) 8)  Please give the name and contact details of the commissioner responsible for medical out-of-hours contract(s) 9) Can you please supply a copy of your latest out-of-hours quarterly performance report against national quality standards?

 Information regarding the award of the GP Led Health Centre within your trust. If available we would like to know details of the award, contact information for the personnel involved with it, and the time frame for the project.

 I would like to request under the Freedom of Information Act answers to the following questions on NHS dentistry within your trust. 1. With how many dentists your Primary Care Trust currently holds NHS dental contracts (in this financial year). a)The total number of dental providers with which you hold UDA contracts; b) the total number of dental practices that are covered under (a); and c) the total number of dental performers that are covered under (a) and (b), d) If available, please could you provide me with the postcode locations of all dental practices that you have listed  2. What is the (a) maximum; and (b) minimum your Primary Care Trust is paying to a dentist in your area doing NHS work in the current financial year per Unit of Dental Activity (UDA). (When providing the maximum and minimum, please could you also state for each how many UDAs are being provided in total under under (b). that particular contract).

 Could I obtain the number of people who were diagnosed with febrile neutropenia and the number of deaths due to febrile neutropenia for 2008?

 Act for the period from April 2007 to March 2008. Each question relates to dentists, dental practices and patients that your PCT has responsibility for. Please can you tell me: The number of NHS dental practices within your PCT area - The number of NHS dentists within your PCT area -The number of patients registered with each dental practice. Please let me know the number at at the end of March 2008. -The number of new patient registrations with each dental practice during 2007/08 -The number of dental practices with open lists, i.e. accepting new patients.  What was your PCTs budget for tackling obesity in 2007, 2008 and the predicted budget for 2009?How many patients has your PCT treated for obesity in 2006, 2007, 2008? What proportion of these patients were under-18 and under-12? Is tackling obesity a major priority for your PCT?What plans has your PCT developed to tackle child obesity?Has the PCT denied any treatments for obese patients?If so, which treatments were denied?Does you PCT offer any weight loss schemes for patients, and at what cost?How much has your PCT spent on reinforcing equipment and adapting facilities in primary care for obese patients?

 I would like to know of any schemes the PCT is offering to help combat obesity.1. I would first like to whether there are any opportunities for free or subsidised gym membership A) If yes, I would like to know how many people receive free or subsidised gym membership. B) I would also like to know who would qualify for such a scheme - is it just open to people who are clinically obese?2. I would like to know how much the PCT has invested on such schemes over the past financial year.3. Finally I would like to know whether the PCT has any schemes in place that offer financial rewards for losing weight.4. If so, how many people are taking part in such a scheme?

How many appointments, of any sort, were cancelled in your PCT due to lost notes each year for the last 5 years? If possible please break responses down into appropriate groupings, e.g. GP appointments and consultant appointments. All eligibility criteria for access to NHS care for assisted conception (including IUI, IVF, ICSI and OD). This should include all of the criteria used in consideration of tertiary care referral. How many cases of Assisted Conception were practiced in your Trust in 2006, 2007 and 2008? How many applications for Assisted Conception were made to your Trust in 2006, 2007 and 2008?

 1.  How many children under 18 are being treated for addiction to; a.  recreational drugs b.  prescription drugs 2.  The number of children under 18 being treated for each drug?  I would like figures for recreational and prescription drugs. e.g. cocaine - 5, ecstasy, 15 3.  The breakdown in percentage terms for each year group being treated for drug addiction. For example; Under 11s; cocaine 15 %, ecstasy, 20 % etc4.  How much money has been spent for each year, over the past three years, on treating under 18s addicted to drugs?
